Client is primarily hiring Coach under his credential as a Certified Master Health and Wellness Coach through AFPA Fitness. Under this credential, Coach has training and expertise in health and wellness coaching, goal setting, values clarification, lifestyle resources, and motivational techniques. The primary function of a coach under that credential is to facilitate a supportive alliance in which Client can maximize personal potential, develop and attain goals, and become independent of Coach. Client understands that about 80% of live session time will fall under this style of partnership.
Client is also hiring Coach as a General Life Advisor who blends his other training and experience to provide holistic wellness services. This training includes but is not limited to yoga, meditation, self-compassion, personal training, assisted stretching, corrective exercise, nutrition coaching, cranial sacral therapy, emotional freedom technique (“tapping”), kinesiology, inner family systems, somatic trauma therapy, and compassionate inquiry. Client understands and acknowledges that these other trainings give Coach a wide lens of expertise to provide on-the- spot interventions and practices, as well as alternative perspectives, opinions, and advice. Client understands that the Coach Report provided in the Embodied Living Assessment is created entirely under this General Life Advisor role, and falls outside the scope of practice for an AFPA Health and Wellness Coach.
Client understands that about 20% of live session time will include Coach-Directed activities that fall under the General Life Adviser role. These include on-the-spot interventions or practices guided by the Coach, as well as opinions, perspectives or advice. Client understands that any of these Coach-directed activities may be unsolicited, and that that Client may freely accept or reject any opinion, perspective, advice, or on-the-spot intervention or practice offered by Coach.
Coach is not acting as a mental health or medical professional - and this service does not involve the diagnosis or treatment of mental, physical, or emotional disorders. This service does not serve as a substitute for mental health or medical treatment.

Coach will:
Deliver a coaching process and structure as laid out in this agreement
Practice according to generally accepted coaching standards and the NBHWC Code of Ethics
Abide by the confidentiality agreement contained herein
Client will:
Communicate openly and honestly with Coach, indicating any preferences or changes desired
Actively create the time to participate fully in the agreed-upon program
Acknowledge accountability for actions, behaviors, and choices
Take responsibility for moving the Embodied Living Assessment Program along, including completing their individual work and scheduling the required programs within reasonable timelines.
Submit a short two-week summary form before each coaching session. A sample of these forms can be found here.

Client and Coach will work together over five sessions to conduct a holistic assessment of Client’s life and to set long-term goals. The format of this service is:
Session 1
Client and Coach will have a 30 minute onboarding meeting, in which Coach will describe the program.
Client Self-Assessment
Client will fill out a holistic life self-assessment, and draft primary goals. This assessment usually takes between 3 and 6 hours. As references, the current versions of this self- assessment can be found here, and the goals form can be found here.
Coach Review
Coach will review Client’s self-assessment, and then e-mail Client to schedule a coaching session.
Session 2
Coach and Client will meet for up to 50 minutes for Coach to ask questions about the self- assessment.
Coach Report
Coach will write an assessment report on the resources and blockages in Client’s life. This report will focus on how thoroughly Client is living in an embodied, authentic way - and suggest ways to find resources or clear blockages on client’s path toward living a more embodied life. This report is not a medical or mental health diagnostic. A sample template of this report can be found here.This may take up to two weeks, and will be e-mailed to Client as both a report and a video recording of Coach summarizing the report. Client Review
Client will review the Coach Report and then schedule a 50 minute coaching session.
Session 3
Coach and Client will meet for up to 50 minutes for Client to ask questions about the Coach Report.
Client Goal-Setting
Client will write a revised version of their goals.
Coach Review
Coach will review Client’s goals, and provide feedback by e-mail. A Sample email feedback cab be found here.
Session 4
Coach and Client will meet for up to fifty minutes to review Client’s goals, and to thoroughly discuss Client resources, obstacles, and social environment.
Client Goal Finalization
Client will write a final version of their goals. A sample version of final goals can be found here.
Session 5
Coach and Client will meet for up to fifty minutes to review Client’s final goals, strategize for how to break those goals into smaller steps, and create plans for then progress is challenging. For clients moving into a 6 month coaching program, Coach and Client will work together to
set two-week goals.

Client assures that they have signed the Liability Waiver for Devin Scott Heals (or will sign it before the first coaching session), and acknowledges that their agreement to the Liability Waiver covers all services in this agreement. In particular, Client understands and acknowledges that Coach is not acting as a mental health or medical professional - and that coaching does not involve the diagnosis or treatment of mental, physical, or emotional disorders. Client is aware that results or specific outcomes through coaching are not guaranteed. Client acknowledges that they are responsible for their own results and outcome. Client agrees to hold Coach free of all responsibility and liability for any adverse situations created as a direct or indirect result of resources, referrals, or advice given by Coach.
